Özet: 1Hacettepe Üniversitesi, Sosyal Hizmet Bölümü The purpose of this article is to mention about problems of chronically ill people to integrate them with the community and about social work practices as a solution. The chronically ill people and their families can be faced with some economical, psychological, familial and social problems. In this article, it is discussed about that these problems affecting their social lives and the roles of social work to eliminate these problems and provide their social integration. The diagnosis of the chronic illness is one of the most critical moment of these people. Chronically ill people can be faced with some problems such psychological, economical, social and familial problems. These problems can be seen as barriers to affect their social integration. From the point of social work profession it can be seen that some professional practices should be carried out to eliminate the problems of these people and to provide their social integration. It is thought that the micro, mezzo and macro level practices by social workers can eliminate the barriers to affect the social integration of chronically ill people
