Alzheimer Hastalığında Ortalama Trombosit Hacmi Düzeyleri Vasküler Risk Göstergesi Olabilir mi?

Özet: Objective: It has been reported that vascular risk factors play an important role in the pathophysiology of Alzheimer's disease (AD) and platelets is effective in this process. Mean platelet volume (MPV) is a marker of platelet activity and it is a physiological variable which has a significance of hemostasis. It has been reported that MPV increases in vascular disease. The aim of this retrospective study is to investigate the relationship between MPV and AD. Method: The study group consisted of 38 patients (the mean age 77.26±8.04) which admitted to outpatient clinics because of memory impairment and who were diagnosed Alzheimer's and 29 healthy subjects (the mean age 76.38 ± 6.79). MPV levels and biochemical parameters (folic acid, vitamin B12, total cholesterol, LDL, HDL, triglycerides, glucose, TSH) were measured in patients and control groups. Mini-mental state examination (MMSE) was performed in patients and control groups. The results were assessed by using Student t-test for parametric data and Mann-Whitney U-test for nonparametric data. Results: MMSE values (16.2 ± 5.78) were significantly lower in AD compared with those of the controls (26.58 ± 2.3) (p <0.001) and MPV levels (10.22±0.72) were significantly higher in AD compared with those of the controls (9.83±0.49) (p <0.05). MPV levels of mid and high levels of cognitive impairment in AD were significantly higher compared with those of mild stage patients (respectively p<0.01, p<0.05). In the group of patients with AD showed a negative correlation between MPV and MMSE (r= -0.526, p<0.01). Discussion: Vascular risk factors play an important role of pathogenesis and progression of AD. We aimed to determine the role of MPV in the pathogenesis of AD, relationship between vascular risk factors and dementia levels in AD and if there is a precursor finding of AD. MPV may be a significant marker to indicate vascular damage in AD.
