Özet: Stenosis or obstruction of the Subclavian artery proximal to the ostium of the Internal mammarian artery can cause lschamia due to the diminished or reversed flow in the target artery in patients with previous coronary artery bypass grafting utilizing internal mammary artery. This phenomena is called Coronary subclavian steal syndrome. We present a patient who had a previous CABG operation utilizing left internal mamarian artery admitted for recurrent angina after 3 years of angina free period. Angiography revealed left subklavian artery stenosis proximal to the Internal mamarian artery ostium which was treated with an antesternal axillo-axiller bypass. Although this phenomenon is a rare entity, we believe It will gain importance with the increasing utilization of the internal mamary artery for coronary artery surgery.
