Özet: Aberrant left subclavian artery aneurysm with right aortic arch is an uncommon anomalious of arcus aorta. Aneurysmatic enlargement of aberrant left subclavian artery (Kommerell"s diverticulum) at the origin from aorta makes this pathology much more complex. The patient, who underwent operation two times in an other centre, admitted to our hospital, with dispnea and disphagia. In this patient aberrant left subclavian artery aneurysm with right aortic arch was diagnosed, and operation was planned. In the operation Kommerell"s diverticulum was repaired by graft interposition in descending aorta and left subclavian artery division and mobilization. In post operative period, there was no symptom. Aortography, performed postoperative 6th month, revealed that integrity of aorta was normal and subclavian artery appeared in the late ejection phase via collaterals.
