OUR THERAPY MODALITY FOR VENOUS THROMBOSIS FOLLOWING SNAKEBITES

Özet: Purpose: A patient with snakebite must be evaluated as complex toxication. In our country bites are generally due to vipers and vessel dinamism changes due to hematoxin frequently causes acute symptomatic proximal vein thrombosis at bited extremity. We aimed to present our cost effective and convenient methods in the patients with this complication beside the general systemic therapy as vascular surgeons. Methods: We studied 36 patients with acute symptomatic venous thrombosis due to snakebite and admitted to ‹zmir Atatürk Education and Research Hospital Emergency Service between 01.01.2000 and 31.12.2002 and transpordet to cardiovascular surgery clinic. Eleven (30.5%) of the cases were women and 25 (69.5%) were men and average age was 48. We found that patients were admitted to emergency service approximately 19 hours after the bite. We performed continuous intravenous standard heparin infusion with APTT control, due to weight, and oral anticoagulation was continued with warfarin sodium. Results: There wasn"t any pulmonary embolus and systemic toxication finding in our patients. All cases were transported to service at the 2nd day of intensive care unit and standard therapy protocol was continued. Early complications of systemic standard IV heparization wasn"t developed. Average hospital stay was 6.2 days. We continued anticoagulant therapy at least 3 months with outpatient clinic control. Cure was achieved with low molecular weight heparin therapy in a patient with recurrent venous thrombosis. There wasn"t any posttrombophlebitic syndrome in 31 cases that could be followed. Conclusion: Aims of the therapy for acute venous thrombosis that was developed after snakebite and evaluated as complex toxication case are; relieving the symptoms, establishing the normal activity of the patients, preventing recurrences, limiting the therapy duration and costs. We thought that one of the cheapest and safest therapy protocols is our method. (Turkish J Vasc Surg 2003; 12(1): 25-30)
