PREDICTION OF RISK FACTORS FOR CORONARY ARTERY DISEASE BEFORE PERIPHERIC ARTERIAL RECONSTRUCTION: LOGISTIC REGRESSION ANALYSIS

Özet: Purpose: In this study, we investigated the incidence and severity of coronary artery disease among the patients undergoing surgical peripheric arterial reconstruction. The strongest predicting factors which can be helpful in the algorythm of indication of coronary arteriography were analysed. Patients and Methods: Among the 180 patients undergoing peripheric arterial reconstruction, 73 patients (40.6%) were referred to diagnostic coronary arteriography for the presence of suspected coronary artery disease (CAD). Risk factors related to CAD, as well as the additional morbidity factors were analysed, by using chi-square test, and logistic regression analysis. The p value, standart deviations, risk ratio, and 95% confidence interval for each variable were determined. All proportions were compared with Fischer"s Exact Test and all p values were two-tailed. Also the angiographic findings and the level of the obstructions were correlated with coronary artery disease. Results: Within the group of 73 patients (to whom the coronary arteriography has been applied), 19 % of the patients had no significant coronary arterial lesion. 22 % of patients had the lesions requiring medical tharapy only. 26 % underwent percutaneous cardiological interventional procedures, and 33 % underwent coronary artery bypass grafting operation, without any complications. Preoperative angiographic findings and the obstruction levels also yielded a significant correlation with the prevalence of CAD. The preoperative variables that predicted CAD by univariate analysis were sex, hypertension, diabetes mellitus (DM) and carotid artery disease. As a result of logistic regression analysis, DM and hypertension were the strongest predictor in the diagnosis of CAD. Conclusions: Especially the diabetic and hypertensive patients, as well as the patients requiring revascularisation procedures in multiple levels require more concern in the referring process to coronary diagnostic procedures.
