Özet: Purpose: Evaluation of the extra-anatomic bypass grafting and its results. Material and method: Early and midterm results of the 32 patients, median age 61.3 years (range 46-81), 2 female and 30 male, having undergone extra-anatomic bypass grafting between January 1990 and January 2004 were investigated retrospectively. This study was done on the patients having undergone axillofemoral, axillobifemoral and femorofemoral crossover bypasses using prosthetic vascular grafts. Indications were; aorta-enteric fistula, infected graft, infected pseudoaneurysm in iliac and femoral arteries and the patients having laparatomy contraindicated iliac artery occlusion. Results: Mortality rate 3.12% peroperatively. Enduration in graft location, hyperbilirübinemia and scrotal edema were most commonly encountered complications. Patients were followed up 3 years on average. Graft occlusion rate was investigated with 3 and 6-month periods postoperatively and later on annually using duplex scanning. These results showed that the patients undergoing the grafts placed in axillofemoral and axillobifemoral bypasses had longer graft intact rate than those who had grafts applied in femorofemoral crossover suprapubic position. Additionally, the first group patients did not have any secondary occlusion. Conclusion: Extra-anatomic bypasses are alternative therapy method and can be performed easily in life-threatening cases; such as; aorta-enteric fistula and infected aortic graft.
