Özet: Purpose: Cerebrovascular stroke which is a serious complication has an important place on the morbidity and mortality after coronary artery surgery. Between January 2000 and December 2004, 58 patients underwent combined coronary and carotid artery surgery. Methods: During 5 years period, combined surgery were applied to 58 patients who were planned for elective coronary bypass surgery. Among 58 patients, 42 patients (72 %) were male and 16 (28 %) female. All patients were followed up in early postoperative period in terms of mortality and cerebrovascular stroke. Results: Surgical therapy was carried out for the left carotid artery in 36 patients and the right carotid artery in 22 patients. While endarterectomy was applied in whole patients, arteriotomy was closed primarly in 10 patients, with saphenous vein in 15 patients and with dacron or ePTFE patch in 33 patients. Carotid artery surgery was performed before coronary bypass procedure in all patients. The mean duration of carotid clamping aortic clamping and total perfusion timesu were 18±5.7 min., 36±13 min., and 69±14min. respectively. Postoperative transient minor neurological deficit was developed in 4 (6.8 %) patients. Mortality and major neurologic sequele was not observed in early the postoperative period. Conclusion: Carotid arteries should be evaluated in patients who are under risk of postoperative cerebrovascular events before coronary bypass surgery. Simultaneous carotid endarterectomy and coronary bypass surgery is a method of choice for patients with critical carotid and coronary artery stenoses. (Turkish J Vasc Surg 2005;14(2):1-6).
