Özet: Carotid artery endarterectomy has been investigated with prospective randomized studies in the span of the last decade. These studies are focused on decreasing the risk of ipsilateral stroke incidents, especially for patients which internal carotid artery is involved in the atherosclerotic stenotic process. On the other hand, studies that deal with the improvement of the best surgical technique for carotid artery stenosis, constitute the keystone of studies on the issue. There are several commonly em- ployed techniques used to repair the carotid artery following endarterectomy. The type of closure after a carotid endarterectomy, primary closure versus patch angioplasty, remains controversial. Most surgeons now routinely use patch angioplasty of the carotid artery as it results in lower rates of long-term re-stenosis and post-operative stroke. Selection of patch material is also controversial, with supporters for the use of both vein patch (saphenous or neck veins) and synthetic patch materials [polytetrafluo- roethylene(PTFE) or Dacron]. Synthetic grafts are essentially equal to vein grafts in terms of recurrent re-stenosis and peri-operative stroke rate. Existing data from randomized controlled trials suggest that the type of patch, whether vein or prosthetic, has no effect on short-term or long-term results. There are not many prospective randomized clinical studies related to Carotid artery endarterectomy, that has a sound scientific constitution. As a common problem, most of these randomized controlled studies are faced with limiting factors. For example, evaluation of re-stenosis involves various technical diffuculties in series that utilize different graft materials. We aim to evaluate patch platy results on carotid artery endarterectomy in light of recent advances in literature. Pubmed is used as the primary database (www.pubmed.com). Keywords used for literature search were, carotid endarterectomy, patch, primary closure, long term results. Mainly, results from prospective randomized trials were utilized for the analysis of comparative data.
