Büyük Safen Ven Varislerinin Tedavisinde Endovenöz Lazer Ablasyon: Üç Yıllık Deneyimlerimiz

Özet: Objective: We present our 3-years experience on endovenous laser ablation therapy in patients with lower extremity superficial venous insufficiency. Material and Methods: The patients enrolled in the study were 258 patients who underwent endovenous laser ablation with 980 nm diod laser (Biolas- 15D) for superficial venous insufficiency after administration to the cardiovascular surgery clinic. Patients were classified using clinical severity, etiology, anatomy, pathophysiology (CEAP) classification before the operation. Venous clinical severity score (VCSS) scores were recorded which depend on scoring the clinical symptoms and findings of patients preoperatively. In total, 258 patients, 309 extremities were treated with endovenous laser ablation procedure under tumescent anesthesia. Patients were clinically examined and screened with colour Doppler ultrasonography at first week, first month, third month and sixth month after the operation. Safenous venous occlusion, recanalisation and residual varicosity were evaluated with colour Doppler ultrasonography. Results: Clinical succes of endovenous laser ablation ther- apy was 97.1%. Echymosis and tenderness was inspected in variable severity due to the procedure in 43% of the patients. Local abcess was seen in 2 patients, cellulitis in 3 patients, tromboflebilitis in 2 patients, parestesis in 4 patients, hematoma in 2 patients and edema in 2 patients, as minor complications. Stabili- sation of all minor complications were obtained with medical therapy. Minor complication rate was 4.5%. There were no major complication as deep venous trombosis. Recanalization was detected in 9 patients during medical examinations. Recanalization rate was 2.9%. A catheheter part was stuck in greater saphenenous vein in one patient after exposing to laser energy. It was extracted with open surgery. Conclusion: Endovenous laser ablation procedure is performed and replaced surgical therapy with high clin- ical success in our clinic as in various other medical centers.
